How we work

Chariot Health works to provide practitioners who do home visit work and our aim is to bring high quality care to you in your own environment.

Our clinical coordinator creates a team in accordance with your requirements. This team would usually include a doctor and a nurse, but may also include additional practitioners such as a physiotherapist,  occupational Therapist or a Counsellor – depending on your needs.

We strive to create a relationship that works for you. This could either mean regular check ups, or visit’s as determined by you.
